１ 調達内容

（６）入札方法 落札決定に当たっては、入札書に記載された金額に当該金額の10パーセントに相当する額を加算した金額（当該金額に１円未満の端数があるときは、その端数金額を切り捨てるものとする。）をもって落札価格とするので、入札者は、消費税に係る課税事業者であるか免税事業者であるかを問わず、見積もった契約金額の110分の100に相当する金額を入札書に記載すること。
（７）本契約においては、資料の提出、入札等を電子調達システムにより行う。なお、電子調達システムにより難いものは、発注者の承諾を得て紙入札方式に代えることができる。
２ 競争参加資格
（１）予算決算及び会計令第 70 条の規定に該当しない者であること。なお、未成年者、被保佐人または被補助人であって、契約締結のために必要 な同意を得ている者は、同条中、特別の理由がある場合に該当する。
（２）予算決算及び会計令第71条の規定に該当しない者であること。
（３）令和7・8・9年度厚生労働省競争参加資格（全省庁統一資格）「物品の販売」において、Ａ、Ｂ又はＣ等級に格付けされ、中国地域の競争参加資格を有する者であること。
（４）次の各号に該当する者は競争に参加できない。
①資格審査申請書または添付書類に虚偽の事実を記載した者。
②経営の状況または信用度が極度に悪化している者。
（５）次の各号に掲げる制度が適用される者にあっては、この入札の入札書提出期限の直近２年間（⑤及び⑥については２保険年度）の保険料について滞納がないこと。
①厚生年金保険 ②健康保険（全国健康保険協会が管掌するもの） ③船員保険 ④国民年金 ⑤労働者災害補償保険 ⑥雇用保険
（６）入札時において厚生労働省から指名停止の措置を受けている期間中の者でないこと。
（７）入札に参加を希望する者は、暴力団等に該当しない旨の誓約書を提出しなければならない。
（８）その他予算決算及び会計令第７３条の規定に基づき、支出負担行為担当官が定める資格を有する者であること。
(９）この入札の入札書提出期限の直近１年間において、厚生労働省が所管する法令に違反したことにより送検され、行政処分を受け、又は行政指導（行政機関から公表されたものに限る。）を受けた者にあっては、本件業務の公正な実施又は本件業務に対する国民の信頼の確保に支障を及ぼすおそれがないこと。
（10） 競争への参加を希望する者は「自己申告書」を令和８年１０月２２日１２時００分までに提出すること。

（１）契約手続において使用する言語及び通貨
日本語及び日本国通貨。
（２）入札保証金及び契約保証金 免除。
（３）入札者に要求される事項 この一般競争に参加を希望する者は、１の（２）に示した調達内容を履行できることを証明する書類と２の
（３）の資格審査結果通知書のコピーを添付して、入札書の受領期限までに提出しなければならない。
また、入札に参加を希望する者は、入札書の提出時に、支出負担行為担当官が別に指定する暴力団等に該当しない旨の誓約書等を提出しなければならない。
入札者は、開札日の前日までの間において、支出負担行為担当官から当該書類に関し説明を求められた場合は、それに応じなければならない。
（４）入札の無効 本公告に示した競争参加資格のない者の提出した入札及び入札に関する条件に違反した者の提出した入札書は無効とする。
また、入札に参加した者が、（３）の誓約書を提出せず、又は虚偽の誓約をし、若しくは誓約に反することとなったときは、当該者の入札を無効とするものとする。
（５）契約書作成の要否 要
（６）落札者の決定方法 予算決算及び会計令第79条の規定に基づいて作成された予定価格の制限の範囲内で最低価格をもって有効な入札を行った入札者を落札者とする。

５ Summary

（６）Qualification for participating in the tendering procedures: Suppliers eligible for participating in the proposed tender are those who shall:
①not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore,minors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be
applicable under cases of special reasons within the said clause;
②not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting;
③have Grade A,B or C “Sales of products”for participating in tenders by Ministry of Health, Labour and Welfare (Single qualification for every ministry and
agency) in Chugoku Region in the fiscal years 2025, 2026, and 2027;
④The following persons are ineligible to participate in the competition:
a Those who have included false information in their qualification application or supporting documents.
b Those whose business situation or creditworthiness has deteriorated drastically.
⑤there not being nonpayment about the premium for most recent two years inadeadline for bid book presentation of this bid about the person whom a system to advocate next is applied to ;
a welfare annunity insurance
b health insurance
c seamen’s insurance
d national pension
e worker’s compensation insurance
f unemployment insurance
⑥not be suspended from transactions by the request of the officials in charge of contract.
⑦Those wishing to participate in the bidding must submit a written pledge stating that they are not affiliated with organized crime groups or similar organizations.
⑧ meet the qualification requirements which the Obligating Officer may specify in accordance with Article 73 of the Cabinet Order.
⑨In the year immediately preceding the deadline for submitting bids for this tender, any person who has been referred to the public prosecutor's office, received administrative sanctions, or received administrative guidance (limited to those made public by administrative agencies) for violating laws and regulations under the jurisdiction of the Ministry of Health, Labour and Welfare will not be likely to jeopardize the fair implementation of this project or the securing of public trust in this project.
